NCP for Appendicitis

Appendicitis is inflammation of the appendix and is a surgical emergency. Appendicitis commonly presents with right lower abdominal pain, nausea, vomiting, and decreased appetite. Appendicitis is caused by a blockage of the hallow portion of the appendix, most commonly a calcified "stone" made of feces. However inflamed lymphoid tissue from a viral infection, parasites, gallstone or tumors may also cause the blockage.

Pain first, vomiting next, and fever last has been described as the classic presentation of acute appendicitis. As the appendix becomes more swollen and inflamed, it begins to irritate the adjoining abdominal wall. This classic migration of pain may not be seen in children under three years. This pain can be elicited through various signs and can be severe.

Nursing Assessment Nursing Care Plan for Appendicitis


1. The identity of the client
2. Medical history : Current medical history; complaints of pain in postoperative wound appendectomy, nausea, vomiting, increased body temperature, increased leukocytes.
3. Physical Examination : Cardiovascular System: To determine vital signs, presence or absence of jugular venous distension, pallor, edema, and abnormal heart sounds. Hematologic System: To determine whether there is an increase in leukocytes is a sign of infection and bleeding, nosebleeds splenomegaly. Urogenital System: Whether or not the tension of the bladder and lower back pain complaints. Musculoskeletal System: To determine whether there is difficulty in movement, pain in bones, joints and there is a fracture or not. The immune system: To determine whether there is lymph node enlargement.
4. Test : Routine blood tests: to determine an increase in leukocytes is a sign of infection.
Abdominal examination photo: to know the existence of post-surgical complications.

Nursing Diagnosis for Appendicitis
  1. Ineffective Breathing Pattern related to the act of anesthetics.
  2. Acute Pain related to the surgical incision.
  3. Risk for Fluid Volume Deficit related to vomiting.
  4. Imbalanced Nutrition: Less than Body Requirements related to anorexia.
  5. Risk of Infection related to surgical incision.
