Nursing Care Plan for Acute Coronary Syndrome

Acute coronary syndrome is a term used for any condition brought on by sudden, reduced blood flow to the heart. It is an emergency. It includes unstable angina and heart attack.

Symptoms of Acute Coronary Syndrome

Chest pain or discomfort may immediately signal to you that something’s wrong with your heart. Other symptoms, however, may leave you unsure of what’s wrong. Take note of these common signs of an acute coronary syndrome:
  • Chest pain or discomfort, which may involve pressure, tightness or fullness
  • Pain or discomfort in one or both arms, the jaw, neck, back or stomach
  • Shortness of breath
  • Feeling dizzy or lightheaded
  • Nausea
  • Sweating
Some additional heart attack symptoms include:
  • Abdominal pain
  • Pain similar to heartburn
  • Clammy skin
  • Lightheadedness, dizziness or fainting
  • Unusual or unexplained fatigue
  • Feeling restless or apprehensive

Nursing Care Plan for Acute Coronary Syndrome

Nursing Diagnosis: acute pain r / t tissue ischemia secondary to coronary artery blockage.

Goal : Pain experienced by patients can be reduced.

Expected outcomes:
- The client states chest pain is gone / controlled.
- Clients can demonstrate relaxation techniques.
- Clients can indicate reduced tension, relaxed and easy to move.

Interventions:

1. Provide a comfortable environment, calm, and give slow activity.
R: Reduce external stimuli where anxiety and heart strain and limited coping abilities and judgment of the current situation.

2. Assist relaxation techniques, such as deep breathing / slow, distraction, visualization, guided imagery.
R: Helps in reducing the pain response.

3. Provide supplemental oxygen by nasal cannula or face mask as indicated.
R: Increase the amount of oxygen available for the use of the myocardium and also reduces discomfort with respect to tissue ischemia.

4. Give appropriate indications such as antianginal drugs, beta-blockers, analgesics.
R: To control pain and increase the comfort of patients so that the healing process runs smoothly.
